It is HUC-JIR policy for all enrolled students to submit medical information that is added to your confidential student record, in fulfillment of certain governmental and insurance requirements, and to assist with any emergencies that may arise. Medical forms are required before matriculation. All incoming students must have the official medical forms complete and submitted to the National Office of Admissions no later than one month prior to the start date of your program. (Current HUC-JIR students may want to update your records). The documents include information for you to review and forms for you to complete with your doctor. You will need to schedule a medical appointment to complete the required information.
If you have a learning disability, please be prepared to provide documentation to the appropriate staff administration on the HUC-JIR campus you attend. This documentation will enable administration to support you for success in your program of study.
HUC-JIR will handle information received from the psychological evaluation (Rabbinical and Cantorial applicants only), medical questions from the application, in addition to these required medical forms, consistent with the requirements of the academic program applied for, applicable law, and relevant HUC-JIR policies. The latter includes, but is not limited to, HUC-JIR’s Learning Disabilities Policy. Access to all medical and psychological health information will be limited to HUC-JIR personnel with a need to access such information as part of their job duties on behalf of HUC-JIR. All medical and psychological information will be kept in a separate applicant medical file that is maintained in a secured (limited access) location.
